GuidingSigns Analytics
GuidingSigns Analytical web based reports with drill down capabilities provides risk stratification of the client’s patient population to identify and categorize patients based on their propensity for poor health outcomes and high costs in the future. The reports also identify patient specific evidence based opportunities for improvement using an exhaustive set of clinical care, behavioral health and medication-related indicators. The areas of focus within the reports include:
- Population profile by level of risk (Critical, High, Medium and Low)

Health Outreach and Targeted Education
This service, performed by a team of telephonic health outreach staff, if offered to employers, health plans, community healthcare clinics and government-funded programs that are specifically seeking population-based improvement in targeted health outcomes. Success in the delivery of this service is measured by patient contact and engagement rates, by measurable improvement in well-defined clinical indicators of health and quality and by reduction in avoidable healthcare expenses. Comprehensive Care Management
This service option covers a broad array of telephone-based health education, patient empowerment and behavior modification services that will be provided to members with chronic illnesses or long-term care needs. This service generates both guaranteed cost-savings and measurable improvements in clinical quality indices. For employers specifically, we will focus on reducing absenteeism and disability leave days, reducing avoidable health expenses and improving overall employee retention and morale. Services will be performed by a multi-faceted healthcare team consisting of physicians, U.S. licensed RNs, health educators and pharmacists. Areas of focus within this category of service include:

Both these service options have certain key elements in common. First and foremost, all telephonic interactions in Altruista Health’s programs, whether for health outreach or for comprehensive care management are customized and tailored to the patients’ social, cultural, linguistic and behavioral traits. Additionally, all education and advice given to patients is strongly rooted in evidence-based clinical guidelines, endorsed by the National Institutes of Health and other reputed national health associations, such as the American Heart Association and the American Diabetes Association. These guidelines are converted into automated scripts within our care system, and are strictly followed during telephonic interactions.
