Care Management

GuidingSigns® Analytics

GuidingSigns is Altruista Health’s clinical decision support system, and includes predictive risk modeling, gaps in care identification tools and assessment analytics. GuidingSigns is embedded directly into the care management system so that any service or education interventions identified at the population or individual member levels can be automatically added to a member’s care plan.

Community Portal

GuidingCare® Community is a robust, web-based care coordination portal developed specifically for use by care providers, support services organizations and other key partners in the care management continuum. The Community portal is designed to function as a shared care coordination system for health plans and external provider organizations.

Member Portal & Mobile App

The GuidingCare® Member Engagement Portal and companion Mobile App enable consumers to securely access health records and care plans, and connect with their care teams at home or on the go.

MicroStrategy® Reports and Dashboards

GuidingCare is integrated with a robust MicroStrategy reporting and dashboarding platform, which includes dozens of standard operational, clinical and financial reports. MicroStrategy also includes ad-hoc analysis and reporting capabilities that allow users to build custom reports and dashboards by connecting with more than 300 clinical care, behavioral health and medication-related indicators.
