Four-year partnership serves frail and elderly
Elderplan is a not-for-profit organization in New York that serves Medicare, long-term care and dual-eligible plans. Most of their 26,000 plan members are frail and elderly. Elderplan has relied on Altruista Health for four years to help reach their care management goals, with a special focus on intense complex care. Altruista has helped Elderplan by automating New York Uniform Assessment Systems for them so that this data is collected efficiently and can be used to develop care plans.
