Medica Selects Altruista Health for Enterprise Care Management Platform

GuidingCare platform to support more than
1 million members in 9 states  

(Sept. 16, 2019, Reston, Va.) – Altruista Health, an innovative provider of care management technology solutions for health plans and provider organizations, today announced that Minnesota-based Medica has chosen Altruista’s GuidingCare® care management platform to serve more than 1 million members in employer, individual, Medicare, Medicare Advantage and Medicaid health plans across nine states.

“We used a comprehensive RFP process to explore innovative approaches and technologies so we can reach deeply into our communities,” said Medica Senior Vice President of health services Nichole White, RPh, MBA. “Medica offers comprehensive, integrated services across the full continuum of care. Medica serves Minnesota, Iowa, Kansas, Missouri, Nebraska, North Dakota, Oklahoma, South Dakota and Wisconsin, with many members located in rural and remote areas lacking internet service. Field clinicians visiting these areas will use GuidingCare’s award-winning Mobile Clinician app to assess members, create care plans, capture signatures and make real-time social services referrals on tablet devices. These devices automatically sync to the GuidingCare platform when internet access is restored.

Social determinants of health shape an estimated 20 percent of a person’s health profile and the figure is even higher for members in public health plans.
