Strong Partnership Supports Duals

Thanks to strong partnership, a Midwestern health plan now has a better way to manage their dual-eligible population, specifically those falling under Home- and Community-based Waivers (HCBS) rules. A collaborative approach between the health plan and its care management partner, Altruista Health, resulted in a robust Service Plan function being built into Altruista’s GuidingCare® technology platform.

A new Service Plan functionality supports care managers at the plan, which serves nearly 2 million members across several states. Service Plans must meet Medicaid guidelines for cost and quality, while affording Duals members maximum independence. This innovative collaboration delivers both.
