CareSource Wanted to Promote Member Engagement and a Whole-person Approach

CareSource is a nonprofit, multistate health plan recognized as a national leader in managed care. Based in Ohio, CareSource administers one of the nation’s largest Medicaid managed care plans and offers a lifetime of access to care through health insurance, including Medicaid, Health Insurance Marketplace, Medicare Advantage and dual-eligible programs. Altruista announced in May 2019 that CareSource had selected Altruista’s GuidingCare platform to manage its nearly 2 million health plan members across five states in Medicaid, Medicare Advantage, Health Insurance Marketplace and dual-eligible programs.

“Our focus on complex populations requires a comprehensive care management approach because of the interdependencies of their physical, behavioral and social health care needs,” said CareSource Chief Clinical Officer Christina Turner. “We looked for a partnership approach and the right technology to deliver administrative efficiency that aligns with our member-centric philosophy.”

CareSource prioritized highly proactive outreach in its RFP process so they could promote member engagement and facilitate a whole-person approach. Altruista’s Mobile Clinician app will put GuidingCare technology in the hands of the CareSource regional care management teams to conduct assessments, create care plans and make social service referrals on Windows and iOS tablets, even in remote and rural areas that don’t have reliable internet service.

GuidingCare has an integrated search-and-referral tool for social service related needs that efficiently connects care managers and members with community partners and agencies that connect members with food, transportation and other resources. These social determinants of health can present steep barriers for vulnerable populations in accessing providers and following through on care plans.

“GuidingCare is a 360-degree member-centric platform that excels at allowing interdisciplinary care teams (IDT) to collaboratively and efficiently manage the continuum of healthy to complex populations to best health outcomes,” said Altruista Chief Executive Officer Ashish Kachru. “The IDT care managers have access to state-of-the-art tools within GuidingCare that nudge them at every step of the way to integrate best practices into managing their members. These best practices have been identified through millions of successful outcomes.”
